need to do, is to complete these forms: Team Form, Code of Conduct/Eligibility
Form, Emergency Medical and Physical forms. You will also need the
meeting handout. Please fill
out forms and return them to Coach Eric:

3 of these forms must be turned in either prior to -- or at
Equipment Night on Oct 23rd. They can be given to Coach
Carstens -
South 142. Equipment can only be given out, if the forms have
been received. After this date
athletes not receiving equipment will be subject to equipment
availability.
Transportation Reminder: For
away meets only,
all swimmers must
use CHS School transportation to-and-from meets. No exceptions.
